Patient History and Presentation:

 

Peter Weaver is a 52 year old gentleman who was involved in an MCA.  He sustained multiple injuries to his head, chest and abdomen.  His wife died in the accident.  Peter had a blood alcohol reading of 0.12 on admission.  He has no medical history and is not on any medications.

 

Peter was admitted to the ICU on 16/5/12 and transferred to the HDU on the 22/5/12.  During his time in ICU, he required mechanical ventilation for respiratory support.  He had an unstable blood pressure and required an Adrenaline infusion.

 

You are team nursing.  Peter is a patient in your care in the HDU. The RN Div 1 allocates you to perform a nursing assessment on Peter.  These are your findings:

 

CNS:    eyes open to speech.  Pupils equal and reacting to light.  Obeys command.Moving all limbs equally.Disoriented to time, place and person.  Appears restless and is trying to get out of bed.Unable to determine pain scale due to confusion.

 

CVS:    Peripherally warm and dry.  No peripheral oedema.  Heart rate 60bpm sinus rhythm.BP 150/90 mmHg.  Temperature: 38.5C.  Arterial line L) radial artery.CVC insitu R) subclavian vein.  Hydration Normal Saline @ 50ml/hr

 

Respiratory:    RR = 28 resps/min.  Wheeze on inspiration. Coughing up green sputum.Short of breath on minimal exertion.Chest tube (ICC) insitu R) lung.Draining haemoserous fluid.On Underwater Seal Drainage.ICC dressing dry and intact.  Dressing changed 21/5/12

 

Renal:  IDC insitu.  Draining 40-80ml/hr.Clear, non-offensive urine.  Inserted 16/5/12.  Fluid

Balance = 1500ml +ve

 

GIT:     Abdomen soft, non-tender.  Bruising at S/C heparin sites.Tolerating diet and fluids.

 

Endocrine: BGL= 5.5mmol./L

 

Musculoskeletal: #R ankle- POP; #R clavicle- sling

 

Skin:    Moist. Good skin turgour. Redness on L)buttock.  No skin tears.  Old bruising from seat belt healing- yellow in colour. Abdominal bruising at S/C injection sites

 

Social: Visited by his daughter and son.  They have come from the funeral of their mother who was killed in the MCA. Peter is involved in local community work and is a volunteer for the CFA, SES and World Vision.

 

Spiritual: Peter and his family regularly attend services at their local Anglican church. Peter has been visited by leaders and members of his church community.  Peter supports various mission organisations and has travelled to some remote communities to voluntarily assist in building projects
