It is an intervention for the health need “lack of access to healthy foods.” 1) In adequate access to healthy food2) Lack of examination facilities for deadly diseases like cancer3) Lack of safety among the residents due to absence of adequate regulating force4) Lack of medical stores5) Limited elder-care homes in the region |
Social Determinants Reflection
- What is that definition of Social Determinants?
Social determinants basically involve conditions within the environments wherein individuals are born, live, work, learn, play, worship and age. These conditions have an impact over a wide array of health, quality-of-life upshots and risks and functioning. Conditions (such as social, physical and economic) within such varied settings and environments (such as school, workplace, neighborhood and church) are regarded as being “place.”
- What is a vulnerable population?
Vulnerable population chiefly takes in patients who are ethnic or racial minorities, elderly, children, underinsured, socioeconomically deprived or ones facing specific medical conditions (Liamputtong, 2007). People belonging to vulnerable population category frequently experience health conditions, which are worsened through needlessly insufficient healthcare. This population involves disadvantaged sub-sector of the society that requires high care, particular ancillary considerations and increased safety.
- How did the Empathy video make you feel?
There is no video so please check and forward
- Give a vulnerable population group name and describe why they are vulnerable.
This particular section addresses one among the high vulnerable group of the society i.e. the elder people. There is no doubt in the fact that the elder people are expected to easily catch any infection or experience health issues. Likewise, these are the ones who are highly impacted as compared to their young n case if the elderly person is physically capable family members. The elder people are regarded as being a vulnerable population, even they are less capable of taking care for themselves
- How does being in a vulnerable population group affect their health?
There is no doubt in the fact that being a vulnerable population greatly affect the health of elder people. This is because of the reason that elder people are more likely to catch infections or become ill as compared to the other segments of the society. Also, elder people require high time to recover from any type of illness or poor health.
- What does ACE’s stand for? Describe how this relates to vulnerable populations and why you think it is discussed in this training.
“ACEs” basically imply Adverse Childhood Experiences (Kitchener et. al., 2013). Such experiences could take in things such as emotional and physical abuse, caregiver mental illness, neglect as well as household violence. The more is ACEs experienced by a child, the more he or she is likely to suffer because of things such as diabetes and heart disease, poor academic accomplishment as well as substance abuse afterwards in their life (Norcross, 2002). There is no doubt in the fact that ACEs is related to the vulnerable population i.e. the children since it involves how children are effected and get ill due to different factors very easily.
- What were your thought when you learned the percentage of people’s health that is affected by social factors?
One cannot deny the fact that large percentage of people health is greatly impacted through social factors (Kitchener et. al., 2013). Social factors, like income, employment, education, community security and social assistance could greatly impact how well as well as how long one lives. Such factors have an impact on the capability of making healthy decisions, afford medical care along with housing, deal with stress and many others.
- Describe one barrier to seeking assistance and what can be done to alleviate or ease that barrier.
One barrier in seeking assistance is the availability of adequate funds to be capable of paying for the offered services. In such a case it is highly essential to make sure that the offered provisions must be easily affordable for all. Everyone should be capable of obtaining the offered services. The services must be offered at a cost that every person could easily afford them
- Describe the Unpacking your bias activity and what you learned about yourself and your biases.
No info provided
- Discuss Motivational Interviewing. What is it and do you feel prepared to have these types of conversations with your patients? If not what is the barrier and how can you overcome that barrier.
Motivational interviewing basically refers to an approach wherein in one becomes a helper during the change procedure as well as expresses acceptance of his/her client (Hall et. al., 2012). It is a manner of interacting with substance-utilizing people, not just like an assistant to other therapeutic practices and a pattern of counseling, which could help in resolving the ambivalence, which stops clients from accomplishing personal objectives (Hall et. al., 2012). I feel I am not ready to conduct such kind of conversation with the client since I am not a good listener. For improving upon my listening skills I would engage in more group discussion activities and self-improvement and enhance them within a short time-period.
- What gets in the way of:
- Helping patients identify and build supports?
- What is an example of something you have done or could do or say to help a patient identify and build supports?
- Helping patients identify and build supports?
Lack of effective communication greatly gets in the way of helping patients identify and develop support. Proper communication could help the patient identify and build support.
- Recognizing your own biases and changing how you are looking at the person or situation?
Holding a stereotype regarding a particular person or situation could greatly get in the way of recognizing own biases and changing how one is looking at the person or situation. Such people are bound to view a person or situation with set mindset.
- Using positive, strengths-based and patient-centered communication?
Lack of proper understanding related to patient’s history and condition gets in the way of using positive, strengths-based and patient-centered communication.
- Really hearing patients…what is important to them, what they want, etc.?
Lack of patience and listening ability really gets in the way of hearing patients i.e. what is important to them, what they want
- What is your self – care plan? How will you avoid burnout?
My self-care plan essentially involves understanding the problem faced by the patient and possible reason for the same. Then, after properly examining the situation offering adequate care to the person. In order to avoid burnout it is highly essential to ensure effective and adequate communication with the patient and offer them a sense of care and security.

Community Practice
The community chosen for this particular task is West Des Moines, IA. The overall population within the area of West Des Moines has increased at a comparatively speedy pace over the period of past 16 years, increasing from 46,403 individuals during the year 2000 to around 64,560 during the year 2016. The ethnic and racial blend of the area is continuously rising. While the white populace even now signifies 85.4% of the overall populace, all minorities have augmented at a rate greater as compared to the average for the region in general. A considerable increase in the Hispanic populace took place that increased by over 108.7% between the period of 2000 and 2010. It has observed that the condition of houses as well as socio-economic status is good in West Des Moines. The issue of homelessness and house disparities is likely to occur in case if proper housing evaluation is not carried out and an action plan needs to be developed for effectively managing the housing issue at West Des Moines. The action plan would involve various steps. First of all, it would involve examining the present situation and the need for additional housing. Secondly, it would require examining the present housing provisions with present population. Thirdly, it would involve examining the need for additional housing in the future times taking into consideration the expected growth in population and need for housing. Lastly, it would involve properly understanding the needs of the people whether they are satisfied with their present housing provisions or looking to shift to some other place.
