It delivers clinical services to more than 700,000 people through seven Clinical Programs from more than twenty-five different location. Its services are located across 2,800 square kilo meters in the east – largest geographical catchment area of any metropolitan health service in Victoria. Social Workers are employed by Eastern Health who provides services to in-patients, out-patients and mental health and community programs. Social Workers are the essential part of the team of specialist health professional at Eastern Health. They see the patients referred by GPs. Social Workers provide assistance for various range of issues, which may include, family conflict, drug and alcohol concerns, social isolation, housing, child or elder abuse”.My Role
In my role as Social Worker, I would deal with various clients with a range of issues including complex family conflict, drug and alcohol, social isolation, housing and access of community resources and services. As a Social Worker at Box Hill Hospital within the Eastern Health network, I need to help people to explore their feelings and emotions that are often related to their experiences. I need to approach clients with different models of practice used in social work in order to understand their circumstances. Those information need to be recorded with confidentially and it can be access in future. I need to make sure when doing counseling with client, they properly understands confidentiality, topic of discussion. I am therefore responsible for preparing case plan and referral of the clients
Client Situation
Nalini was brought into the hospital unconscious after overconsumption of alcohol. Her husband called an ambulance in order to take her to emergency care. After treatment Nalini is physically fit but she has various anxieties about family issues. Therefore GP has sent her to me in order to counsel her for alcohol and family conflict. She came to me in very distressed state. Her husband does not want to live together so he wants her to move out of the house. Her husband does not want her to be around their daughter in her current stage but Nalini does not want to leave her husband. Apart from that, she is having difficulty to sleep and reporting feel guilty and worthless. Nalini indicates that she wants to reduce the alcohol consumption and if possible to stop.Client Background
Ms. Nalini is 40 years old. She has been consuming alcohol on and off for over 2 years. Nalini is working as a Senior Manager in a National Company which is based in Melbourne. She is married. She said that her job is very stressful. As a part of her job, she works long hours and has to attend various meetings and work functions. She mentioned that she began drinking since 2 years ago in order to cope with the pressure from the work and social discomfort when she attends different kinds of work functions.
Feelings, Values and Hopes from the Client
Nalini is very intelligent women. She is a good mother and loves her family unconditionally. She has good insights about what she is doing at present and past. She is confident about herself that she can stop using drugs and stop Heroine.Nalini is feeling very distressed about what has happened. She wants to be together with her family and live happily. She has a feeling that her family will accept her once she admits to stop drinking alcohol and stop taking Heroine. She is angry by herself because she spoils her image in her family but she believes that everything will be all right.
Nalini has high hope to re unite with her settled happily once again. This confidence is very important for her in order to get rid of negative aspects of her life.
Significance of Other People and Institutions
Based upon the interview conducted by me with Nalini, it is well stated that Nalini is overwhelmed, anxious & stressed out. This mainly occurs due to the alcohol problem. The main reason for her anxiousness is that, her husband does not like her to drink & spend hefty money on the same.
She is close to her husband & loves him very much. She wants to get rid of this problem as early as possible. In order to give her family & husband due time and respect she tried to cut down the number of drinks she used to take after office hours. She had cut down her drinks from 4 glasses to 2 glasses. She also tried a day without a drink. At the same time, she says it was very difficult for her to control herself at the end of the day.Nalini feels that, once she will quit or reduce the number of drinks she would be able to devote more amounts towards her family. Her relationship with the family including her husband would be better as compared to the present situation. This will also decrease the tension between our relationships.
I would address the problem of Nalini by making a key change within her attitude. This would be made possible when, Nalini decreases the consumption of beer & she quits drinking. She will be able to show better attitude towards her husband & family. This would also lead to decrease in the levels of anxiety & stress.
Part 2Based upon my interview with Nalini, it has been regarded that Nalini has come at the Box Hill Hospital since she is feeling overwhelmed, anxious & stressed out. It has been noticed that, Nalini is suffering from an alcohol problem. Her husband is strictly against alcohol & does not like her drinking too. Nalini’s husband collapses on her spending too much money over drinks. Even after suffering from this problem, she is quite confident regarding the fact that she will try to make a change & reunite wither family.
Based upon the interview, it is seen that Nalini’s drink intake is approximately 4 glasses after work. She admits the fact that, she has been drinking after she joined work & was unable to cope with the work stress. After work, she is supposed to attend various functions, meeting which makes her too tired. Nalini has been married for the past 2 years from now. Her husband is good but was unable to understand how important it was for her to unwind after the long working hours. She has tried to give her family time & due respect by cutting down the number of glasses from 4 to 2 in a day. But, this made her day very difficult. She admits that, if she stops or reduces the number of glasses then her relationship with her husband would be perfectly fine. Her husband would think that, she is willing to do many things for her family & decrease the levels of tensions. She specified the fact that, she would decrease the number of beer which would change her attitude towards her family.
In the end I communicated Nalini that she was making the right kind of effort to decrease the consumption of beer or alcohol. Based upon the interview, it has been seen that you have the desire to build better relations with your husband in the future years to come. These words of mine has helped to boost up Nalini’s morale as well as help her to come out of this situation.

Critical Reflection
IntroductionSolution based therapy (SBT) along with the narrative therapies shall be used in case of Nalini which would help them to reduce the stress & anxiety levels. Solution based therapy refers to a psychotherapy approach to which would help Nalini to solve its problem relating of overwhelmed, anxious & stress level. This therapy would help in order to explore the various problems of Nalini. This therapy would add value as a preliminary & would be used safely within the other treatments.
Apart from the solution focused therapy, narrative therapy shall also be used in case of Nalini. One of the main therapies which shall be kept in mind in order to reduce the levels of anxiety refers to Cognitive Behavioral Therapy (CBT). This type of a therapy would be quite helpful in Nalini’s case to reduce the levels of anxiety.
Analysis and Critique
The cognitive behavioral therapy (CBT) would help Nalini to change the various principles which would help her to influence the cognitive process. The cognitive behavioral therapy (CBT) would help Nalini to realize the fact that excessive consumption of liquor would be termed highly maladaptive in order to cope with the various other problems as mentioned above in the write up. It has been observed that Nalini is unable to cope up with her job as well as her family. Due to over stress of work Nalini was unable to maintain a balance between her husband & her professional work. Unable to cope up between the two has led to high consumptions of alcohol.
